How to Access the Medical History Form

The Medical History Form can be accessed Via both the 'Treatment' section and the 'Patient' section. 

Treatment Section

Go to the 'Treatment' section > 'Medical History'

Patient Section

Go to the 'Patient' section > 'Clinical' > 'Medical History'. See below:

The Medical History Questionaire can be filled out in either in the reception with receptionists or in the treatment room with the clinician by following the steps provided above. 

Alternatively, the Medical History Questionaire will also be automatically sent to the patients through the Patient Portal If your practice has this additional feature as a part of your subscription then patients can access this form prior to the appointment saving time for both the patient and the clinician when they are in the practice.
