How to Download a Design & Sign Form

The option to download completed/signed forms through Design and Sign is available by following the steps outlined below.

Simply navigate to either the Treatment or Patient section > Design & Sign tab > Select the Preview button. See example within the Treatment section:

From here, you can select 'Download' and this will download the Design and Sign form in .pdf format to the selected location:

Patients will also have the option to download both unsigned and signed Design and Sign forms, medical
history questionnaires and treatment plans on the patient portal.
