Within Aerona, there are two ways that you can send forms. Through the 'Treatment' section for clinicians and via the 'patient' section to give the flexibility for receptionists to also be able to generate forms to patients.
To generate a form via the 'Treatment' section, simply select the 'Design and Sign' tab. Alternatively, you can generate a form via the 'Patient' section by selecting 'Patient' > 'Design & Sign' > 'Form'. Both sections will direct you to the below screen. See below:
You will then be able to use the drop-down facility to select the form that you would like to generate and select ‘generate’. If you are finding it difficult to find the form that you have created you can also search the forms by typing in the name of the form. See below:
Simply select 'generate' to generate the form. Once you have selected to generate the form the preview of the form will appear. See below:
You can also add a comment into the form if you would like by selecting 'Add a comment'. A comment can be a great personal touch that can provide direction and give the patient a more understanding of what they are being asked to complete.
Once you are happy to proceed with sending the form, simply select 'Send to XXXX'. See below:
Once you have sent the form you will be able to view all forms that have been previously sent to the patient within the same section. See below:
You will be able to view their status; the date sent and also preview the form that was sent. If you have sent a form incorrectly you can also cancel the form by selecting ‘cancel’.
Patients will then be able to complete the form via the practice portal or the patient portal.
