Form templates can be added by following the below steps:
Go to 'Admin' > 'Design & Sign' > ' Form Template'. See below:
To view any current form, simply select on the form and this will bring up its content.
To create a new form all you need to do is right-click the ‘forms’ folder and select ‘create’ >’new form template’. See below:
Once you have created the form you may want to rename it. This can be done by right-clicking on the template and selecting ‘rename file’. See below:
This options menu will also give you the option of removing the file by selecting ‘remove file’ if you no longer require this consent form.
The next step would be distinguishing this form template into one of the two categories ‘patient assessment’ or ‘patient survey’. This can be done by using the drop-down facility just above the header of the form and selecting the appropriate category. See below:
Once you have selected the category you can now go ahead and add the questions. To do this simply select ‘click here, to start adding content’. See below:
You will then be able to add in the question that you would like to ask, the question response type, and add any validation if this is required. You also have the option of adding a placeholder if applicable, a placeholder is temporary information that is displayed that describes the expected information that should be added to the field. You can also make the question a required question. Simply select ‘Save’ to add the question. See below:
Once you have selected ‘save’ your question will then appear on the form. The icons to the right of the form give you the option to delete the question, edit the question, and move it. See below:
Simply follow the same process to add other questions to your form.
Once you have added in each question the form will automatically be saved to the system. If the practice is multi-site the form will also be shared across to the other location/s.
